Website Terms of Use

These Terms govern access to and use of aoscenter.com, its related subdomains, and AOS online services that link to these Terms.

The Site supports the AOS enterprise, which includes legally separate health care providers, ambulatory surgery centers, educational organizations, and affiliated service entities operating under the AOS and AOSC brands.

Additional agreements may apply to particular services. When a separate agreement applies to a particular transaction or service, that agreement controls to the extent it conflicts with these Terms.

1. Acceptance of These Terms

These Website Terms of Use ("Terms") govern your access to and use of aoscenter.com, its related subdomains, and any AOS website, landing page, form, survey, calendar, chat feature, educational platform, or other online service that links to these Terms (collectively, the "Site").

By accessing or using the Site, you agree to these Terms and the AOS Enterprise Website Privacy Policy. If you do not agree, do not use the Site.

Additional agreements may apply to particular services, including medical or dental treatment; ambulatory surgery-center services; financial responsibility and payment; telehealth; clinical credentialing and privileges; courses, conferences, events, and continuing education; patient portals; financing or payment plans; and third-party products or services.

2. Enterprise Structure and Responsible Entity

The Site serves multiple legally distinct organizations operating under the AOS and AOSC brands. Certain pages, forms, educational content, products, and services are offered by different legal entities.

Users should refer to the applicable page, agreement, invoice, consent, estimate, registration, or other transaction materials to determine the responsible entity for a particular service or transaction.

The entities currently supported by the Site include:

  • AOS Institute, LLC — education, courses, conferences, events, training, publications, and related activities
  • AOS Miami, PLLC — medical and dental practice
  • AOS ASC, LLC — ambulatory surgery center
  • Alfi Oral Surgery, PA — medical and dental practice
  • Airway Outpatient Surgical Services, LLC — ambulatory surgery center
  • AOS Orthodontics, PLLC — medical and dental practice
  • Any additional entity properly added to the Site and identified in the applicable page, form, agreement, or notice

The use of a shared brand, website, technology platform, administrative system, or common ownership does not merge these entities or make each entity responsible for the acts or obligations of every other entity.

3. Informational Purposes Only

Unless expressly stated otherwise, Site content is provided for general informational and educational purposes only. Site content does not constitute medical advice, dental advice, diagnosis, treatment recommendations, anesthesia advice, legal advice, financial advice, insurance advice, or a substitute for an evaluation by an appropriately licensed professional.

Do not delay seeking professional care because of information appearing on the Site.

4. No Doctor-Patient or Provider-Patient Relationship

Accessing the Site, reading its content, submitting a form, requesting an appointment, sending a message, completing a survey, or communicating through a general website channel does not by itself create a doctor-patient, dentist-patient, surgeon-patient, or other professional relationship.

A clinical relationship is established only when an appropriate AOS health care provider or facility accepts the individual for care and any required evaluations, consents, registrations, or agreements have been completed.

A request for an appointment, consultation, estimate, procedure, or service is not accepted until it is confirmed by the responsible provider or facility.

5. Emergencies and Sensitive Information

The Site, general email addresses, online chats, text messages, contact forms, and social-media accounts must not be used for emergencies. For a medical emergency, call 911 or go to the nearest emergency department.

Unless a form is specifically identified as an authorized secure clinical form, do not submit detailed medical records, highly sensitive health information, Social Security numbers, full payment-card information, passwords, government identification documents, or other information unnecessary for the stated purpose of the form.

Email and ordinary electronic communications may be delayed, intercepted, misdirected, or accessed by someone with access to your device or account. Email should not be considered a secure method of transmitting sensitive health information unless specifically designated as secure by AOS.

6. Privacy, HIPAA, and Health Information

Use of the Site is governed by the AOS Enterprise Website Privacy Policy.

Certain AOS entities are health care providers or facilities subject to the Health Insurance Portability and Accountability Act and other health-privacy laws. Other AOS entities, including AOS Institute, LLC when acting solely in its educational capacity, may not be HIPAA covered entities.

Information submitted while seeking or receiving health care may become protected health information when created, received, maintained, or transmitted by a covered AOS entity or its business associate. Such information is governed by the applicable Notice of Privacy Practices and health-privacy law.

General website information is not necessarily protected health information merely because the Site concerns health care. Use of the Site does not constitute a HIPAA authorization, consent to treatment, or acknowledgment of a Notice of Privacy Practices.

7. Permitted Use of the Site

You may use the Site only for lawful, personal, professional, or legitimate business purposes consistent with these Terms.

You agree not to:

  • Violate any applicable law or regulation.
  • Attempt unauthorized access to the Site, accounts, servers, networks, or data.
  • Circumvent security or access controls.
  • Introduce malware, malicious code, or harmful material.
  • Interfere with Site performance, availability, or security.
  • Scrape, crawl, copy, harvest, or extract Site content or data through unauthorized automated means.
  • Impersonate another person or entity.
  • Submit false, fraudulent, incomplete, or misleading information.
  • Use another person's contact or health information without authority.
  • Use Site content to train an artificial-intelligence system without written authorization.
  • Use the Site to send spam, unlawful marketing, or abusive communications.
  • Infringe intellectual-property, privacy, publicity, or other rights.
  • Use the Site in a manner that could harm a patient, visitor, AOS entity, provider, employee, or third party.

We may suspend or restrict access when we reasonably believe the Site is being misused.

8. Appointments, Calendars, and Service Requests

Appointment and consultation availability may vary by provider, facility, service, location, insurance status, clinical appropriateness, and scheduling capacity.

Online calendar availability is informational and subject to change. An appointment is not confirmed until accepted or confirmed by the applicable office, provider, or facility.

Submitting an appointment request does not guarantee appointment availability, acceptance as a patient, eligibility for a procedure, insurance authorization, insurance coverage, a particular provider or facility, a specific surgical date, or a particular outcome.

Appointment confirmations, cancellation requirements, deposits, rescheduling rules, and no-show policies will be communicated separately and may vary by entity, location, provider, and service.

9. Deposits, Payments, and Financial Responsibility

Any deposit, cancellation, rescheduling, payment, refund, financing, or financial-responsibility terms applicable to clinical services will be governed by the financial policy, estimate, treatment agreement, payment authorization, or other documentation provided by the responsible health care entity.

Any terms applicable to an educational course, conference, event, membership, publication, or product will be governed by the checkout terms or program-specific agreement presented by AOS Institute, LLC.

Online payment processing may be provided by third-party payment processors. AOS Institute, LLC currently uses Stripe for certain educational transactions, and AOS clinical practices may use Helcim for certain practice transactions. AOS does not store complete payment-card information on the Site. Payment processors may collect and process information under their own terms and privacy practices.

Nothing on the Site guarantees insurance coverage, reimbursement, eligibility, authorization, or payment by a health plan. Patients and purchasers remain responsible for amounts owed under the applicable financial agreement and law.

AOS will not charge a stored payment method merely because a person accepted these general Website Terms. Any recurring, stored-card, or balance-payment authorization must be separately obtained where required.

10. Ambulatory Surgery-Center Services

AOSC Houston and AOSC Miami are separate facilities with separate operational, legal, billing, regulatory, and licensure information.

AOSC Miami is currently planned and completing its initial AHCA licensure process. Nothing on the Site should be interpreted as stating that AOSC Miami is licensed, open, operational, or currently providing services unless AOS expressly updates the applicable page after authorization.

Services associated with a surgical episode may be provided and billed separately by the ambulatory surgery center, surgeon or treating practice, anesthesia professionals or groups, pathologists, laboratories, radiologists, medical-device suppliers, and other independent practitioners or organizations.

These professionals and organizations may not participate with the same insurance plans as the facility or treating surgeon. Patients should contact the applicable entities regarding estimates, billing, and network participation.

11. Estimates, Price Transparency, and Insurance Information

Any price, charge, estimate, benefit, coverage, or reimbursement information presented through the Site is informational and may not reflect the final amount owed.

Actual charges may vary based on services actually provided, clinical circumstances, supplies, implants, medications or devices used, practitioner and facility charges, anesthesia time, pathology, laboratory or radiology services, insurance benefits, deductibles, copayments and coinsurance, medical-necessity or authorization decisions, and other factors.

A nonpersonalized estimate is not a guarantee of final charges or insurance payment. Facility-specific estimate and price-transparency information should be reviewed on the applicable Houston or Miami page.

12. Clinical Privileges and Professional Inquiries

The Site may allow physicians, oral and maxillofacial surgeons, dentists, pediatric dentists, and other eligible professionals to submit an inquiry regarding clinical privileges or professional collaboration.

Submitting an inquiry is not a formal credentialing application unless expressly identified as one; does not create employment, an independent-contractor relationship, or another contractual relationship; does not constitute credentialing, approval, case acceptance, or authorization to schedule or perform cases; does not guarantee that the applicant will receive a formal application; and does not create any obligation to review or approve an application.

All applicants must satisfy the applicable credentialing, verification, privileging, facility-policy, committee, and governing-body requirements before providing services.

13. Educational Services, LearnWorlds, Courses, and Continuing Education

AOS Institute, LLC may provide educational content, courses, conferences, webinars, workshops, publications, training, certificates, memberships, or continuing-education activities.

Educational services may be delivered through third-party learning-management systems, including LearnWorlds or successor platforms. Access to those services may also be governed by the platform provider's own terms of use and privacy practices.

Educational content is not a substitute for professional education, independent clinical judgment, applicable licensing requirements, supervision, facility policies, credentialing, clinical privileges, or compliance with governing professional standards.

Participation in an educational program does not guarantee competence to perform a procedure, licensure, certification, credentialing, clinical privileges, employment, reimbursement, a particular professional result, or acceptance of continuing-education credit by a particular board or organization.

Program-specific enrollment, payment, cancellation, refund, attendance, assessment, certificate, recording, intellectual-property, and continuing-education terms may be presented separately. Those terms control for the applicable program.

14. Telehealth and Virtual Communications

If telehealth, video consultations, or other virtual services are offered, additional telehealth consent and location-specific requirements may apply.

You acknowledge that remote evaluation has limitations; technology may fail or communications may be interrupted; an in-person examination may be necessary; telehealth may not be appropriate for every condition or service; and telehealth may be available only when the provider is authorized to practice in the patient's location.

Do not use telehealth or electronic communications for an emergency.

15. Email, Telephone, and Text Messages

By providing contact information, you authorize AOS to respond to your inquiry and send communications reasonably related to the interaction, subject to applicable law and any consent language presented with the relevant form.

Communications may include appointment information, administrative notices, form requests, patient instructions, billing communications, educational program information, clinical-privileges communications, and other messages you requested or authorized.

Where you separately consent to automated or marketing text messages, message frequency may vary; message and data rates may apply; consent is not a condition of obtaining health care or making a purchase except where permitted by law; reply STOP to opt out of the applicable program; and reply HELP for assistance.

Opting out of marketing messages may not prevent legally permitted nonmarketing, transactional, clinical, security, or administrative communications.

Mobile opt-in information and consent records will not be sold or shared with third parties or affiliates for their own marketing or promotional purposes. Information may be disclosed to service providers that assist in delivering the messaging program and are restricted from using it for independent marketing.

16. Artificial Intelligence and Automated Tools

Certain Site features or AOS workflows may use artificial intelligence, machine learning, chatbots, automated decision support, or other automated technologies to assist with scheduling, communications, education, content organization, customer service, or administrative functions.

AI-generated or automated output is provided for convenience and may be incomplete, inaccurate, or inappropriate for a particular situation. It is not medical or dental advice and is not a substitute for professional judgment, clinical evaluation, or human review.

No automated tool should be used for emergencies. AOS may modify, suspend, or discontinue automated features at any time.

17. Images, Before-and-After Content, and Renderings

Images appearing on the Site, including before-and-after photographs, illustrations, simulations, animations, anatomical graphics, and educational images, are intended for informational or illustrative purposes and do not guarantee individual results.

Individual outcomes vary based on patient-specific factors, the treating professional, the procedure performed, and other circumstances.

Architectural renderings, floor plans, conceptual images, furnishings, equipment depictions, and facility visualizations represent design intent and may differ from the completed facility.

18. User Submissions

You are responsible for information, documents, images, comments, reviews, messages, and other materials you submit through the Site.

You represent that the information is accurate to the best of your knowledge; you have authority to submit it; it does not violate another person's rights; it is not unlawful, threatening, defamatory, deceptive, or malicious; and it does not contain unnecessary sensitive information.

Submitting material does not transfer ownership of your medical records or personal information. Our use of personal information remains governed by the Privacy Policy, applicable Notice of Privacy Practices, consent forms, authorizations, and law.

Do not submit a testimonial, photograph, video, patient story, or other material for public use unless the appropriate written authorization or release has been completed.

19. Accounts and Login Credentials

Certain Site features, including educational platforms or portals, may require an account.

You are responsible for providing accurate registration information; maintaining the confidentiality of your password and credentials; restricting unauthorized access to your device and account; and promptly notifying us of suspected unauthorized use.

We may suspend or terminate an account when reasonably necessary to protect users, the Site, AOS, or third parties, or when these Terms or applicable program rules are violated.

Closing an account does not necessarily require deletion of records that must be retained for legal, regulatory, clinical, educational, contractual, security, fraud-prevention, or archival purposes.

20. Intellectual Property

The Site and its content — including text, photographs, videos, graphics, illustrations, logos, trademarks, trade dress, course materials, surgical education, downloads, page designs, software, and compilations — are owned by or licensed to the applicable AOS entity and are protected by intellectual-property laws.

Except as expressly authorized, you may not copy, reproduce, modify, distribute, republish, display, perform, sell, license, create derivative works from, use for artificial-intelligence training, record, or commercially exploit any Site content without prior written permission.

Course materials, lectures, surgical techniques, presentations, videos, and downloadable educational resources may not be recorded, reproduced, distributed, or used for AI model training without prior written authorization.

You may print or download a reasonable amount of publicly available material for personal, noncommercial use, provided you do not remove copyright, trademark, or proprietary notices.

"AOS," "AOSC," associated logos, and related names and branding may not be used without authorization.

21. Social Media

AOS may maintain accounts or pages on social-media platforms. Interactions through Facebook, Instagram, LinkedIn, YouTube, X, TikTok, or another social-media platform do not create a provider-patient relationship and should not be used for emergencies or submission of sensitive health information.

Social-media platforms operate under their own terms and privacy practices. Public comments or messages may be visible to others.

22. Employment and Vendor Inquiries

Submission of employment information does not create an employment contract, guarantee an interview, or guarantee employment. Any employment relationship must be established through separate written documentation.

Vendor, supplier, partnership, referral, sponsorship, or business-development inquiries do not create a purchasing obligation, exclusivity arrangement, partnership, agency relationship, or other contractual commitment unless separately agreed in writing by an authorized representative.

23. Research, Publications, and Scientific Content

Scientific publications, research summaries, clinical commentary, lectures, and educational materials are presented for informational and educational purposes and should not be interpreted as individualized medical recommendations.

References to studies, techniques, devices, or outcomes do not guarantee that a particular approach is appropriate for a specific patient or that the same result will occur in another setting.

24. Third-Party Websites, Platforms, and Services

The Site may link to, embed, or integrate with third-party services, including maps, videos, payment processors, learning-management systems, patient portals, scheduling systems, social-media services, insurance resources, price-transparency tools, review platforms, customer-relationship-management systems, communications platforms, marketing-automation tools, and other external websites or technologies.

Third parties operate under their own terms and privacy practices. AOS does not control and is not responsible for third-party content, availability, security, accuracy, or practices. A link or integration does not necessarily constitute endorsement.

25. Website Availability and Changes

We may modify, suspend, remove, or discontinue any Site feature or content at any time.

We do not guarantee that the Site will always be available, access will be uninterrupted, content will be error-free, information will always be current, defects will be corrected immediately, or the Site will be free from viruses or harmful components.

Facility information, clinician information, insurance participation, services, hours, personnel, educational offerings, and policies may change.

26. Disclaimer of Warranties

To the maximum extent permitted by law, the Site and its content are provided on an "as is" and "as available" basis.

AOS disclaims all warranties, express or implied, concerning the Site, including implied warranties of merchantability, fitness for a particular purpose, title, and noninfringement.

Nothing in these Terms limits any rights that cannot lawfully be waived.

27. Limitation of Liability

To the maximum extent permitted by law, the AOS entities and their owners, officers, directors, managers, employees, contractors, clinicians, agents, and licensors will not be liable for indirect, incidental, special, exemplary, punitive, or consequential damages arising from or related to use of or inability to use the Site; reliance on Site content; unauthorized access to communications or accounts; third-party services; transmission delays or failures; or loss of data, revenue, opportunity, or goodwill.

These limitations do not apply where prohibited by law and do not limit liability arising from professional health care services to the extent such liability cannot lawfully be limited through website terms.

28. Indemnification

To the extent permitted by law, you agree to defend, indemnify, and hold harmless the applicable AOS entities and their owners, officers, directors, managers, employees, contractors, and agents from claims, damages, liabilities, costs, and expenses arising from your unlawful use of the Site; your material violation of these Terms; information or material you submit without authority; or your infringement of another person's rights.

This section does not require a patient to indemnify a health care provider for the provider's own professional negligence or conduct where such an obligation would be unlawful or against public policy.

29. Governing Law and Venue

Because the Site supports legally separate entities and services in multiple states, the governing law and venue may depend on the responsible entity and applicable service.

Unless a separate agreement states otherwise, matters involving a Florida entity or Florida service will be governed by applicable Florida law; matters involving a Texas entity or Texas service will be governed by applicable Texas law; and matters involving AOS Institute, LLC will be governed by the law and venue identified in the applicable educational or transaction agreement.

Nothing in this section limits rights or remedies that cannot lawfully be waived.

30. Severability and Waiver

If any provision of these Terms is determined to be invalid or unenforceable, the remaining provisions will continue in effect to the fullest extent permitted by law.

A failure to enforce a provision is not a waiver of the right to enforce it later.

31. Changes to These Terms

We may revise these Terms to reflect changes in law, technology, Site features, vendors, services, or business practices.

The "Last Updated" date identifies the current version. Unless otherwise stated, revisions become effective when posted.

Your continued use of the Site after the effective date of revised Terms constitutes acceptance of the revised Terms. Material changes may be communicated through the Site or another appropriate method.
