Terms of Service – AOS Center

1. Acceptance of Terms
By accessing or using the AOS Center website (“Site”), you agree to these Terms and all applicable laws. If you do not agree, do not use the Site.

2. No Medical Advice / No Doctor–Patient Relationship
Content on this Site is for informational purposes only and does not constitute medical advice, diagnosis, or treatment. Use of the Site does not create a doctor–patient relationship. A relationship is formed only after you are evaluated by a provider and appropriate consents are executed.

3. HIPAA and Privacy
AOS Center is a covered entity under the Health Insurance Portability and Accountability Act (“HIPAA”) and maintains safeguards for protected health information (“PHI”) in accordance with applicable law.

  • Information submitted through general website forms, email, or standard contact channels may not be secure and may not be HIPAA-compliant. Do not submit sensitive medical information through unsecured channels.
  • PHI is collected, used, and disclosed only as permitted by law and as described in the AOS Center Notice of Privacy Practices.
  • Use of third-party services (e.g., payment processors, messaging platforms) may involve separate privacy practices outside AOS Center’s control.

4. Use of Website
You agree to use the Site only for lawful purposes and not to:

  • Attempt unauthorized access to systems or data
  • Interfere with Site operation or security
  • Provide false or misleading information

5. Appointments, Deposits, and Cancellation Policy

  • Certain appointments (including consultations) require a non-refundable deposit at the time of booking.
  • Deposits are applied toward the scheduled service but are otherwise non-refundable.
  • If you cancel or fail to reschedule at least 24 hours prior to your appointment, the deposit is forfeited.
  • Late arrivals, no-shows, or same-day cancellations may result in forfeiture of the deposit and may require a new deposit to rebook.

6. Payments and Financial Responsibility

  • All payments for services, consultations, and procedures are final unless otherwise stated in writing.
  • Fees are due as specified at the time of booking or service.
  • AOS Center does not guarantee insurance coverage or reimbursement. Patients are responsible for all charges regardless of insurance determination, including deductibles, co-insurance, and non-covered services.
  • You authorize AOS Center to charge any payment method on file for outstanding balances related to your care, to the extent permitted by law.

7. Ambulatory Surgery Center (ASC) Services
Services performed in the ambulatory surgery center may be billed separately by the facility, surgeon, anesthesia providers, or other entities. You may receive multiple bills for a single episode of care.

8. Telehealth and Electronic Communications
If telehealth or virtual consultations are offered:

  • You acknowledge limitations of remote evaluation and agree that in-person evaluation may be required.
  • Electronic communications may not be secure, and delays may occur. Do not use electronic communication for emergencies.

9. SMS Communications and Consent
By providing your phone number, you consent to receive SMS/text messages from AOS Center for scheduling, reminders, and administrative purposes in compliance with the Telephone Consumer Protection Act (TCPA).

  • Message frequency varies. Message and data rates may apply.
  • Consent to receive SMS messages is not a condition of purchase or treatment.
  • You may opt out at any time by replying “STOP.”
  • For assistance, reply “HELP” or contact the office.
  • AOS Center is not responsible for delayed or undelivered messages.

10. Intellectual Property
All content on the Site (text, images, videos, branding) is owned by AOS Center or its licensors and is protected by applicable intellectual property laws. Unauthorized use is prohibited.

11. Continuing Education
Any educational products or courses offered through AOS Center are subject to separate terms and conditions provided at the time of purchase. If a conflict exists, those specific terms control.

12. Third-Party Services
The Site may link to or integrate with third-party services. AOS Center is not responsible for their content, security, or policies.

13. Limitation of Liability
To the maximum extent permitted by law, AOS Center is not liable for indirect, incidental, or consequential damages arising from use of the Site or reliance on its content.

14. Indemnification
You agree to indemnify and hold harmless AOS Center from claims arising out of your use of the Site or violation of these Terms.

15. Modifications
AOS Center may update these Terms at any time. Continued use of the Site constitutes acceptance of any revisions.

16. Governing Law
These Terms are governed by the laws of the State of Florida.
